This Notice explains how medical information about you may be used and disclosed and how you can obtain access to it.
You may tell us whether to share relevant information with family, close friends, others involved in your care or payment, or disaster-relief organizations. Most marketing, any sale of PHI, and most uses of psychotherapy notes require written authorization. We do not currently use PHI for fundraising.
We may use and disclose PHI to treat you, run our practice, obtain payment, communicate about appointments, work with laboratories and ordering providers, and provide information to a credentialing authority at your direction or as permitted by law. We may also use or disclose PHI for legally permitted public health and safety activities, abuse or neglect reporting, research, organ donation, coroners or funeral directors, workers' compensation, health oversight, law enforcement, government functions, and court or administrative proceedings.
We follow stricter federal or Illinois protections when they apply, including protections for mental health and developmental-disability records, HIV-related information, genetic information, records concerning certain minor-consented services, and substance use disorder patient records.
To the extent we have substance use disorder patient records subject to 42 CFR part 2, we will not use or disclose information in those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and subpoena as required by law.
We must protect the privacy and security of PHI, notify you of a breach when required, follow the Notice currently in effect, and avoid uses or disclosures not described in the Notice unless you authorize them or law permits them.

These Terms apply to this Website and the merchant mariner physicals, mobile specimen collection, and on-site CPR training promoted here. The full Terms presented during booking govern the service.
Website content is general information, not medical advice. Sending an inquiry does not create a clinician-patient relationship or confirm an appointment. If you have a medical emergency, call 911 or go to the nearest emergency department.
An appointment is confirmed when Carepatron or Enhanced Health issues a confirmation. Services promoted on this Website are cash pay. The confirmed price controls. Laboratory testing may be billed separately by the laboratory.
Please provide at least 24 hours' notice to cancel or reschedule an individual appointment. There is currently no late-cancellation fee. A no-show is charged the full scheduled visit price.
Collection requires a valid provider order or eligible lab kit. Enhanced Health does not order tests, perform testing, interpret results, or manage results through this service line. No more than two venipuncture attempts are made per visit. The $99 visit fee remains due if collection cannot be completed.
One $49 reattempt may be scheduled within 14 days for the same order or kit in the service area. A confirmed Enhanced Health collection, labeling, packaging, or handling error receives one no-charge recollection. Recollections caused by a laboratory, provider, kit, carrier, patient preparation, or another outside circumstance are $49 within 14 days and otherwise charged at the regular rate.
Collection begins at age 3. A parent or legal guardian must consent and remain present throughout a minor's appointment.
Mariner examinations document findings accurately for credentialing. Enhanced Health does not guarantee medical qualification, form acceptance, waiver approval, credential issuance, or any decision by the U.S. Coast Guard.
On-site training requires at least six paid participants. A deposit equal to the six-participant minimum, $390 for AHA BLS for Healthcare Providers or $450 for AHA Heartsaver Pediatric First Aid CPR AED, reserves the date and is applied to the class total. Final headcount and balance for additional participants are due 72 hours before training. One reschedule is allowed with at least seven days' notice. Cancellations made fewer than seven days before the class and group no-shows forfeit the deposit. Individual absences do not reduce the confirmed balance. Certification requires successful completion, and each organization must verify course acceptance.
You must provide accurate information, follow preparation instructions, and provide a safe service location. Carepatron, laboratories, carriers, credentialing authorities, and other third parties have their own terms and responsibilities. The Website Privacy Policy governs Website information, and the Notice of Privacy Practices governs PHI.
Illinois law governs these Terms, and Cook County courts are the designated venue where legally permitted. Nothing in the Terms waives patient rights or limits liability that cannot lawfully be waived or limited.
